my first surgery
i had just assisted a surgery about an hour ago. i wont put it as assisted. i think i was juz like a penyebok there. seriously.
it was total abdominal hysterectomy with salpingoopherectomy. (removal of uterus, the tube, and the ovaries)
my patient had ovarian tumour at the right ovary. and i saw the tumour. thank God it was benign looking (based on what the surgeon said la)
yaa… it doesnt look as horrid as what a cancer will look like. how should i describe it ek, it was as big as telur burung unta. and was white colour and ada selaput yang ada banyak blood vessels.
because my patient had two previous caesarian section, we found a lot of adhesion, which make the surgery a bit more difficult. and there were a lot of bleeding vessels at the bladder that my surgeon need to carefully stop the bleeding. stiching the vaginal vault pon doesnt look easy.
owh, im not gonna talk about technical thing here.
i wanna go back why i described myself as penyebok at the operating table. as this is my first day, i was blurr. nasib baik one of the MA show me the way to scrub. i actually already practiced bit n bit. but the hardest part for me was to put on glove. it has always be my problem.
the surgeon n medical officer were nice. maybe im too slow too blurr the surgeon become grumpy. i feel quite down la. coz i was not able to help much. menyusahkan ada la. haihh… why la we have to hav this kouta system. (actually its good. exposure b4 you become a houseman)
i will feel this feelings for a few days until i managed to get over this.
and keep on telling myself this;
“ye nana. its not because of you. memang surgery tadi susah sket.”
owh, plez someone comfort me. huhuh.

Portfolio Forever
Portfolio forever? iye. feel like im forever doing portfolio. haihh.
its not easy to write a portfolio
still writing its not the hardest part
in history and currently, portfolio has done his evil job well. there are couples who break off juz because they ‘berebut’ portfolio case. four years of friendship is nothing compared to 20 pages portfolio which sometimes is never going to be discussed. people dont talk to each other anymore. some bitching around juz because someone else did what they actually did too. which is? curi case. then, the one with stolen case go curi other’s case. haha. some make rules, but never stick to the rule. some very good at reminding others, but also good at giving him/herself excuses to break the rule that they pledged upon. some hold and hold the cases. tried to find best case to write, taking other people’s chance of writing a case.
sound so stupid.

Alhamdulillah (Thank God) I had my case. Tho i dont know what am i going to write. Huhuh. But, InsyaAllah (God Willing), I’ll make this thru. May Allah show us the way.

Untitled~
okay. i havent think of any suitable title for this post yet since it has been quite sometimes since i really write something. been busy lor. internal medicine posting was really busy, compared with other posting. coz we had case presentation every morning. then, in the afternoon, we had management tbl since there are so many things to cover. internal medicine is just too broad!! however, i do love the tbls, when my friends presenting. they were great!! i can see their effort doing the presentations and it does help a lot. tho sometime i had short attention span. hihi.. sorry..
the busyness was not only because of int med, but the coming professional exam. the extra classes, the extra studying. (owh, as if i did extra studying) to be honest, i didnt study much yet, or spent any specific time to study other posting. i just steal some of the times here and there to study things which suddenly just pop up in my mind. besides, i enjoy discussing wif friends from other posting to refresh my memory and learn from them.. =)
today is considered my last day of semester 9. (ouch, it hurts!!) (yeah, if u dont consider the hunted exam la) after this, im gonna have two weeks of study break and then 11 february, my exam will start. im going home for a few days. celebrating chinese new year!! hehe.. then will be coming back to practice csu and concentrate more on study.
haihh.. this exam is really killing me. deep down, i am so scared. im afraid that i wont be able to make it. im not ready. really, there are so many things i havent covered. its like i have to study back all the thing ive learnt in the past 4 and a half year. ok la. not that much la. shuld narrow down to the common2 things only la. huhuh.
i hope we could all work together, hand in hand and pass the exam together and meet up again in Batu Pahat. Yeah. i can feel tears coming down my cheek cause my hope really ‘menggunung tinggi’, as high as the everest. and these are the things which keep me moving on. hope and hope. and the smilling faces i would see when we see each other again in BP campus in March. See you, guys!! =)

patient oh patient
During the past three weeks posting, i have seen quite a number of patients and gained some experience from encounter with them.
1. A 19 year old Muslim boy working at the airport presented to me with episodes of haemoptysis (batuk berdarah) for one night (so, asal dia masuk surgical ward?). he had loss of conciousness (LOC) for four hours after that. (he is still alive now, k) he claimed that he was previously well, he’s a chronic smoker. (didnt ask the alcohol part still 19 and Muslim plak tu) ask him few times. was it batuk or muntah darah? he said its batuk. ok lah. so, me went to look at the bedhead ticket of the patients.
Diagnosis: He had pangastritis secondary to alcohol abuse.
Conclusion; nextime should ask everything necessary, no matter what’s the race or religion or age or sex.
But, proud of myself coz i did cirtically asses the patient’s truthfulness about his LOC. ;D
2. A 56 year old female came in for second cycle chemotherapy for rectal carcinoma. She looks so lonely, so I just approached her. Yeah, I was right coz she couldnt stop talking when she started. Actually, chemo’s patient can come at daily basis for their chemo. Its 5 days for one cycle. However, her house is quite far, so she was admitted. Only some family members come during visiting hour and her children are actually working far away. She told me how was her feeling when she was diagnosed with the cancer. She felt very sad and it was very painful (the heart and the abdomen). However, she accepted it as Ujian from ALLAH. Then, came the issue about stoma. Initially she did not want to have it, coz she thought its dirty and how is she going to pray when she has the stoma? Thus, she seek advised from alim people. She felt relieved as the stoma will not be a hindrance for her to continue beribadah.
She told me,
“Bila ALLAH kasi ujian kat kita, mesti ada jalan penyelesaiannya. Dan ujian itu tanda sayang Dia pada kita. Supaya kita selalu ingat Dia and minta tolong hanya padaNya.”
Conclusion : I was touch by this aunty. She’s such a strong and optimistic person. I wish her well always and lives happily. and Alhamdulillah, she has no side effects of chemotherapy. =)
From my patient, I also teringat the holistic approach that Dr. Loh (Family Medicine) always emphasized on. We should not only treat the cancer, but help her psychologically and spiritually.
Besides, this is a reminder for me to increase my knowledge about Medicine in Islam as there may be other issues related to procedures or treatment which may cause uncertainty when it is related to our life as Muslim.
Ive got two more patients i want to write about. but exam is coming, so need to study. might continue some time later. =)

FAP
today only i found out that FAP(familial adenomatous polyposis) is actually quite common in Malaysia. This week, ive seen three patients. Two of them has had pancolectomy (all the colon has been removed) coz when they come to age of fourties, the polyps in the colon will turn to be cancer. so, u prophylactically removed the colons as to avoid colonic ca. in FAP, there will be more than 100 polyps. patient may be symptomatic or asymptomatic. usually, they may have diarrhea which consist a lot of mucus, n there might also be fresh blood.
As I follow Miss J’s ward round today, i have learnt about the pancolectomy (removal of the large intestines) and reconstruct the bowel as she explained to her patient. She had also explained the complications of the procedure.
I really feel sorry about the patient. She’s just 26. She has strong family history of FAP and colon ca. So, after the colonoscopy she previously had, the surgeon suggested that she undergo pancolectomy. She might have an ileostomy for 6 weeks after the surgery.(the last part of small bowel, bring to lower abdomen and open up through skin and into a bag to let the faeces out) besides that, the surgery itself has its own complications. and they might injured her anal sphincter that later she might not be able to control when to defaecate or pass flatus.
there are a few other things that Miss J has explained and I couldnt wrote everything here coz i need to study too la..
I pray that the patient would be well during and after the surgery and her quality of life will not be affected by her condition. Amin…
ps; Syukur Alhamdulillah (Thank GOd) coz im healthy.. =)

surgery surgery~~
Today is the second day of surgery posting. Fuhh.. how different it is from anaes n radio. im loving it, tho! =)
My first day was quite hectic, as usual, started wif briefing by dr. subash patil n our beloved datuk Kanda. My classmates commented that datuk looks more friendly and smiled to us more. There was no stern face anymore. Yeah!! he told us that the timetable belong to us, so its up to us what we want to do. we can even throw the time table! wo!! haha.. yaya.. with that power he gave us on our time table, come la the great responsibility. huhu… (cam spidey la..) later, in the afternoon we have case presentation wif Prof Lionel. im always love him, tho he is never lost the sarcastic part of him. One of it was
“Are u guys sure u r sem9 students?!”
Itulah.. coz we cant answer the physiology of bile and byk lagi la.. but, salah siapa, kan? huhu.. then, he never let us go back wif tangan kosong. sure got issues to think of and read about.yes, objective IMU tercapai. lecturer stimulate the students to learn.
I miss my surgical patients. Dont know why la, i got very interested when it come to surgery. but, doing surgery? hmm.. still not sure la. today have practiced suturing, ok2 la.. wish i can make a nice suture on a real patient one day nanti. (^_^)
aish.. ok la. need to study for tomorrow’s tbl. its about trauma. haihh.. i kinda dont like trauma. scary sehhh…

Final Year
Hello, friends!!
Its been quite some time since i last blog. had my elective and selective for two months. huh, how i miss those two months.
My final year in Med school has formally started last 2nd September wif Gynaecology posting. I am seriously anxious wif the coming semester 9 final exam since i know how much not prepared i am. huhuh. However, it is yet quite interesting, add up wif being in gynae posting. Lecturers are treating us like a future doc. Of course it come wif higher expectation.
next week will be the fourth week of this posting, hence come the clinical exam and SAQ. Wish that by this weekend i am well prepared, tho im currently far from that. haha.
All right.. Wish all of us good luck.. n hope its not too late.. Happy Berpuasa! =)

This is Ptyriasis Versicolor (fungal infection of the skin). Ape kaitan ngan spaghetti n meatbalss? aha. Teka la!!
I just read that when u do KOH (potassium hydroxide) test, the fungal will appear like spaghetti and meatball. uhuuhu…
medicine ni memang cam tu la.. semua pon nak samakan ngan food. bread n butter la.. kidney bean la, banana la… ;D
ps; owh!! nasib baik sye tak suka spaghetti. hihi

No Work
iye je i dont have any work to do. exam coming another two weeks. MCQ and OSPE for 4 postings, ie; accident&emergency, ophthalmology, otorynolaryngology and dermatology. huhu… where do i got time to study? yayaya. if dont waste my time spending on this karut thingy, sure la i got tme.
okay, nana!! berusahalah!! Em!!
=) good luck, everyone!! then we can enjoy our holiday, elective and selective before stepping on stressful semester 9!!
